| On a daily basis we receive numerous forms and reports for letters regarding patients and their medical conditions that are not covered on the NHS.
These forms are generating a considerable amount of work. There is a charge attached to the completion of these forms in order to cover the workload that they generate.
The government demand that a number of these services are eligible for VAT.
Upon presentation of a form at reception our receptionists will provide you with a written estimate of the cost of the form or request (verbally if a telephone encounter).
Once the form is completed it will be scanned onto the appropriate medical record.
Should you wish for a receipt this can be issued at reception.
Our fees are based on (although lower than) the nationally recommended fees from the British Medical Association.
Typical fees
- HGV/PS/Taxi medicals (30 minutes) £70 + VAT @ £12.25
- Large insurance forms approximately a side of A4 £25
- Smaller insurance forms approximately half a side of A4 £15
- Passport signing £15 + vat @£2.63
- Driving licence £15 + VAT @£2.63
- Private sick note £10
- Holiday cancellation certificate £15 + VAT @ £2.63
- Fitness to travel certificate/letter £15 VAT @ £2.63
- Private letter minimum £18.50 + VAT @ £3.24
- Yellow fever vaccination and certificate £40.
