If you have chosen to register with our practice then please consider arranging the appropriate registration documents as soon as is convenient. Please do not wait until you are unwell as this will save a great deal of stress and potential anxiety from having to commence the paperwork at a time when you may be feeling ill.

If you have any documentation such as a previous medical card then this is helpful but not essential for the registration process. Once registered, your previous medical records are retrieved from your pervious doctor, this is so that we can have a full and meaningful medical record.

Registration forms may be collected from reception and once completed, signed and returned to the surgery then you are registered with Stalbridge Surgery for your General Practice needs.

Alternatively we have produced an online registration form which allows us to ease the process for you. Once completed you are registered with Stalbridge Surgery.

The surgery is operating a six month alternate policy of registration, from 1/1/08 with Dr Clayton and from 1/7/08 with Dr Furber. Essentially this is an administrative exercise as any registered patient is free to see whichever Doctor or Nurse they prefer. However, should you feel strongly about being registered with either of the other Doctor's please send us a separate note, via our general contact form, requesting this.
