Stalbridge Surgery

Dear ......................

Following today’s consultation you have had a blood test or have been asked to have one performed. You can have a blood test by making an appointment with one of our practice nurses. We take our blood tests in the mornings (prior to midday) because they are sent to Dorchester for analysis each day.

The purpose of this form is to:-

  • Clearly inform you of what test your Doctor has requested
  • Act as a check list for what tests have been taken
  • Act as an aid to obtaining the results of all the tests taken.

Results can be obtained by contacting the surgery in an afternoon, or by using the consultation form on the website addressed to the requesting health professional. Please allow at least 3 days for the results to have arrived and been seen by the Doctor.

Practice Information Leaflets are available to fully explain some of these tests:-

  • Cholesterol
  • Prostate Specific Antigen
  • Diet sheets for raised glucose or cholesterol.

ESRerthyrocyte sedimentation rate. It is a non-specific measure of disease activity. It may help the Doctor to know which further tests to perform.

Cholesterol – a high level of cholesterol is one of the risk factors for a number of other problems e.g. Heart Disease, Strokes.
Glucose – Measures your blood sugar and may indicate diabetes.
PSA – prostate specific antigen. Measures prostate activity and may be raised if your prostate is enlarged. It is not a specific test for cancer of the prostate. If you have had this test please ask for our practice information leaflet.
